APPLICATION FORM GUIDELINES

Guidelines for nominating a candidate or candidates to the Selection Committee of the Canadian Lacrosse Hall of Fame.

Please complete pages 1 and 2 of the application form which is available for download (see below), providing as much information as possible about the candidate. Please type or print legibly. If there is insufficient space provided, please use another sheet of paper, and make sure that you number each section to correspond with the item number in the application form.

Section A - should be completed in its entirety. If the candidate is deceased, please provide the name and address of the Next-Of-Kin. Also, please provide birth place of the candidate. Information on Nominator is required to make your submission valid.

Section B

1. The "Field Player" category was suspended after the 1986 Induction year, and has been re-opened effective the Year 2000.

2. "Sports History". Please list, if possible, dates played with various teams, teams played with, leagues played in, etc. All information that is pertinent to the candidate's playing career or building career.

3. "Championships or Equivalent Major Events". List all Championship teams, dates and trophies won (Mann Cup, Minto Cup, etc.), and any awards that may have been won by the teams the candidate played or coached with, etc.

4. "Personal and Team Records Held". Include scoring titles, team titles, etc. Note number of goals, assists and penalty minutes, individual trophies won, etc.

5. "Official Appointments in Sports (National, Provincial, etc.)". List any position(s) held under direction of any level of Government such as the CLA, Sports Committees, or League positions such as WLA, OLA or BCLA. This section is not limited to Lacrosse.

6. "Brief General History of Candidate". Include any information you may consider interesting that will enable the Selection Committee to know the candidate better. Under this heading, also include any participation with Sports other than Lacrosse.

RULES AND REGULATIONS FOR ELECTION TO 
THE CANADIAN LACROSSE HALL OF FAME
NEW WESTMINSTER, BC

1. RULES OF SELECTION ESTABLISHED

These Rules and Regulations are promulgated by the Canadian Lacrosse Hall of Fame Committee (hereafter referred to as the Governing Committee), which Committee was established under the Agreement entered into between the Canadian Lacrosse Association and the City of New Westminster for the management and control of the Canadian Lacrosse Hall of Fame and which Committee has the power to amend their provisions and whose interpretations of their provisions shall be final.

2. CLASSIFICATION OF MEMBERS

There shall be five classes of Membership in the Lacrosse Hall of Fame -Box Section, Veterans Section, Field Section, Builders Section and Team Section.

3. ELIGIBILITY FOR ELECTION

a) Any person who is, or has been, distinguished as a Player or as a Lacrosse Executive or Coach or as a Referee, shall be eligible for election as an Honoured Member of the Canadian Lacrosse Hall of Fame.

b) Player candidates must have concluded their careers as active players for a minimum of five (5) years before being nominated for their election.

c) Candidates in the Veterans' category must have concluded their careers as active players prior to 1965. In the year 2005, eligibility will be advanced to 1970 and, thereafter, will be advanced in five year increments every fifth year.

d) Builder candidates may be either active or inactive at the time of their election.

e) The Governing Committee may induct a team each year based on outstanding achievement in a single season or domination over a specific period of time.

f) The Governing Committee may, at its discretion, induct one player each year in the Field Lacrosse category.

g) Members of the Governing Committee, the Player Selection Committee and the Veterans Committee are eligible for election as Honoured Members.

4. PLAYER SELECTION COMMITTEE ESTABLISHED

a) The Governing Committee shall appoint two Player Selection Committees, one in Eastern Canada and one in Western Canada. Each Committee shall consist of a Chairperson, a Secretary and three additional members who are knowledgeable in the history, traditions and skills of lacrosse and whose prestige, judgement, and integrity are most likely to ensure the fair and impartial assessment of the qualities of the candidates submitted for their consideration and to assure the maintenance of a high standard of excellence in the selections to be made by them.

b) In the appointment of the Members of the Player Selection Committee, consideration shall be given to ensuring representation on a broad geographic basis, with due regard to the interests of lacrosse as well as the many playing eras from which the candidates will be nominated.

c) Appointments to the Player Selection Committee shall be on an annual basis but they should be made with the object of securing maximum continuity of composition of the Committee.

d) Appointments to the Player Selection Committee may be made at any time to fill vacancies occasioned by resignation, death or unavailability. In the event that a Member of the Committee is temporarily unavailable, a substitute may be appointed by the Governing Committee on an ad hoc basis.

e) Player Selection Committee Members may serve for a maximum of five consecutive years. They may be re-appointed following the absence of a three-year period.

5. BASIS OF SELECTION OF CANDIDATES

Candidates for election shall be chosen on the basis of playing, ability, sportsmanship, character, their contribution to their team and to the game of lacrosse in general.

6. NOMINATION OF CANDIDATES

a) Each Member of the Player Selection Committee may make not more than three nominations for each election proceeding.

b) Nominations shall be made in writing and must be filed with the Secretary by April 30th of the current year.

c) Nominations shall contain the fullest possible data concerning the record and the merits of each candidate nominated.

d) Recommendations for candidates may be made by persons other than Committee Members by complying with the provisions of this Regulation. However, no such recommendation shall be considered by the Player Selection Committee unless it is included by a member as one of his/her personal nominations.

e) The Secretary shall, not later than May 15, send to each Member of the Player Selection Committee and to the Chairperson of the Governing Committee a copy of each Nomination and recommendation received and all supporting material, together with the names of their respective nominators.

f) Nominations of candidates for election as Builders shall be made only by the Members of the Governing Committee. Such nominations shall be made in conformity with the Regulations herein set out for the nomination of Player candidates and such nominations, together with all supporting material, shall be sent to all Members of the Governing Committee only, not less than 30 days before the date fixed for the election of candidates.

g) Nominations of candidates for election into the Veterans category shall be made by special Eastern and Western committees of one or more members as appointed by the Governing Committee. Lists of nominees will be submitted to the Governing Committee which will then select the inductees.

h) The Chairperson of the Player Selection Committee (with respect to Player nominations) and the Chairperson of the Governing Committee (with respect to Builder nominations) may request from the nominator, the Secretary or any other knowledgeable person, further investigation and report on any candidate nominated, which information shall be made known to the other Members of the appropriate Committee before election proceedings take place.

7. METHOD OF ELECTION

PLAYER CANDIDATES

a) The election of candidates shall take place prior to June 30th of the current year.

b) The quota of candidates to be elected each year may be two from the East and two from the West, unless otherwise directed from the Board of Governors.

c) A player nominated may be considered for a period of three years. If unsuccessful he/she may be re-nominated by a Selection Committee Member only.

d) A maximum of 15 players shall be considered in the current year.

e) The Chairperson shall preside and the following order of business will be observed so far as possible:

(i) Announcement of quota of candidates to be elected.

(ii) Submission of roster of candidates certified by the Secretary as having been duly nominated in accordance with Section 6 of these Regulations, and that they are eligible for election under Section 3 of these Regulations. No other candidates shall be considered without the unanimous consent of the Committee Members.

(iii) The Chairperson will report any objection filed to any candidate by the Governing Committee or any Member thereof.

(iv) Consideration of candidates and supporting material.

(v) Balloting:
- Secret Ballot shall be used.
- Nominees shall be rated in order of preference from one to eight
Example: First choice receives vote 1.
Second choice receives vote 2.
Third and successive choices receive vote 3 - 8.
- The two nominees receiving the best rating (the lowest number) shall be deemed to be elected.
- In the event of a tie, a run off election between the tied candidates will be held with the winners declared by simple majority.
- The power of election of player candidates is vested in the Selection Committee and its elections are final and not subject to any further consideration.

VETERANS CANDIDATES

a) A veterans category will initially be restricted to players who participated in box lacrosse between 1932 and 1965.

b) The 1960 eligibility proviso will be advanced to 1970 in the year 2005.

c) Thereafter, the eligibility dates would move upward in five year increments in the years 2010, 2015, 2020, etc.

d) One player from Eastern Canada and one player from Western Canada may be inducted into the Veterans category each year thereafter.

e) Eastern and Western nominating Committees, comprised of one or more persons, each initially prepared lists of ten potential inductees: five from each list constitute the Charter Members with the remaining names on both lists forming the eligibility lists for the following induction year.

f) In each successive year thereafter, new names will be added to each eligibility list in order to maintain the lists at five.

g) The Veterans category Nominating Committees are to be independent of the existing Player Selection Committee.

h) A Veteran candidate's name will remain on the eligibility list for 3 years.

i) Balloting shall be by secret ballot and nominees shall be rated in order of preference from one to five. (Example) First choice receives 1, second choice receives 2, third and successive choices receive 3 to 5. The nominee receiving the best rating (the lowest number) shall be declared elected. In the event of a tie, a run off election between the tied candidates will be held, vote for one only and the winner will be declared by simple majority.

j) Final determination of the inductees will be made by the Canadian Lacrosse Hall of Fame Governing Committee.

k) A nomination may be considered for a period of three years.

BUILDER CANDIDATES

a) The election of candidates shall be made by the Governing Committee and may take place at any regularly convened meeting of the Governing Committee, of which due notice is included in the Agenda; provided that advice of the nomination of the candidate to be considered and the supporting material has been filed with the Secretary not less than sixty days before the date of the meeting and has been circulated to all committee Members not less than thirty days before such meeting. Such meeting to occur no later than June 30th of each year.

b) The Governing Committee shall regulate it's own voting procedures based on the number of eligible candidates. Where there are two or less candidates a secret ballot shall be taken voting for the single candidate of your choice. Where there are 3 or more candidates they shall be rated in order of preference from one to the total number of candidates. (Example) First choice receives 1, second choice receives 2, third and successive choices receive 3 to 5, etc. The nominee receiving the best rating (the lowest number) shall be declared elected. In the event of a tie, a run off election between the tied candidates shall occur and the winner shall be declared by simple majority. 

c) A nomination may be considered for a period of three years.

TEAM CATEGORY

a) Selection of a team for recognition in the Hall of Fame shall be made each year by the Governing Committee.

b) The team so honoured must have attained a high level of achievement and brought recognition to the sport of lacrosse at the national or international levels.

c) The team so honoured could represent either the field or the box variations of lacrosse.

d) The team so honoured would be recognized for its outstanding achievement in a single year or it may be chosen for its contribution and/or domination of the sport over a specific stated period of time.

e) Consideration will be extended to teams at the Senior or Junior levels of Field and Box Lacrosse and to teams representing Canada in International competition.

FIELD CANDIDATES

a) The field category was suspended after the 1986 Induction year.

b) At the time of closure, it was stipulated that the category could be reopened if the name of an outstanding player were presented for consideration.

c) A resurgence of the field game has prompted Governors to remove the suspension, effective in the year 2000.

d) Field candidates must have concluded their active playing careers a minimum of five years prior to induction consideration.

e) One candidate may be inducted each year.

f) Final determination of the successful inductee shall be made by the Governing Committee and the vote shall be by simple majority unless there are three or more candidates in which case they shall be rated in order of preference from one to maximum number of candidates. (Example) First choice receives 1, second choice receives 2, third and successive choices receive 3, 4, 5, etc. The nominee receiving the best rating (the lowest number) shall be declared elected. In the event of a tie a run off election between the tied candidates shall take place and the winner declared by simple majority.

8. CERTIFICATION

a) When a person or team is elected as an Honoured Member of the Lacrosse Hall of Fame, the name of such person or team shall be duly certified by the Chairperson for official enrollment in the Lacrosse Hall of Fame and all necessary actions shall be taken for inclusion of the records, souvenirs or other mementos concerning such elected candidate for display in the Canadian Lacrosse Hall of Fame.

b) Should a person for any reason refuse to be so honoured, his name and all pertinent information shall still be enshrined in the Canadian Lacrosse Hall of Fame; the right to honour such an individual is that of the Canadian Lacrosse Hall of Fame and shall stand irrespective of personal feelings of the inductee.

9. PUBLICATION OF ELECTIONS

The Chairperson of the Governing Committee, or some other Member of either Eastern or Western Selection Committee specially designated for the purpose, shall release the election results for general publication.

10. PROCEEDINGS CONFIDENTIAL

No Member of the Governing Committee, either Player Selection Committee or the Veterans Committee shall divulge or disclose the results or other particulars of the balloting in advance of the official release provided in Section 9, nor shall he/she disclose the vote of any Member at any time.

The Canadian Lacrosse Hall of Fame began honouring box lacrosse superstars in 1966 with the induction of twelve Charter Members and six initial selectees. Since then, there have been an additional 125 individuals inducted in the Box Lacrosse Player Category.

There is a general agreement that many other former players desevedly belong in the Hall but, for one reason or another, they have been overlooked. As the years pass - 1997 is the 66th year that lacrosse moved from the field to an enclosed box - the greatness of the game's early stars fade from memory. To permit their contributions simply to vanish would be a disservice to Canada's National Summer Game.

The Governing Committee of the Canadian Lacrosse Hall of Fame, recognizing the need to preserve the lustre of yesteryear, has established a Veterans category in the Box lacrosse sector.

Ten players - five from Eastern Canada and five from the West - will be inducted into the new Veterans Category in 1997; thereafter, one nomination from the East and another from the West will be inducted each year.

Initially, the Veterans Category will represent players who were active between between 1932 and 1960. The 1960 proviso will be moved to 1965 in the year 2000 and, thereafter, will advance in five year increments every fifth year.
